Contractual Agreement I certify that I am the owner of this pet. I grant permission to New Harmony Road Country Inn to obtain veterinary care in an emergency. This boarding facility agrees to exercise all due and reasonable care to prevent injury or illness to my pet. However, in the event of illness or injury, I will not hold the owners and employees of the boarding facility personally liable. I agree to pay all the charges the day I pick up my pet, and I understand that my pet may not leave the premises until all chargs are paid in full. I understand that any animal left for ten days beyond the estimated date of pick-up will be considered abandoned. ______________________________________________ Pet Owner/Date

Notes: -Pets can only be released to owners unless prior arrangements have been made at the time of boarding. -There is a $25.00 charge (plus the day's boarding) for any off hour drop off or pick up. -All pets must be in good health, free of fleas, and clean at time of boarding. We reserve the right to bathe and deflea any animal that has fleas at owner''s expence. We suggest that all pets be on heart worm medication and flea preventative. -We pride ourselves in providing absolutely the best and most comfortable stay for your pet. Our staff consists of family only. Working are Bill and Debbie Phelps, our son John Phelps II, and our daugher Traci Noel.
